> 86.1 % Breastfeeding initiation rate achieved in the local agency, compared to 68 % in the region and 77.8% in the state. - HPP:
> Completion of the 2013-2017 Community Health Assessment and Community Health Improvement Plan. The CHIP identified two priorities for community focus based on community input and data analysis: Promoting Mental Health and Preventing Substance Abuse is one priority, and Preventing Chronic Disease, the second. The CHIP is built around continuing efforts in these areas and suggests new ideas to help the community to improve its overall health. - CHS:
> Successful NYSDOH surveys of the Childhood Lead Poisoning Prevention and Licensed Home Care Services Agency nursing programs.100% participation and implementation of the state mandate requiring Influenza prevention measures of vaccination and face masks during the flu season.
